Forms for public library staff
These forms are for the use of Queensland public library staff to submit enquiries to the State Library of Queensland. Individuals wishing to contact the State Library of Queensland should refer to the Contact us page for further details.

- Author / title request for purchase new window, external service - Request the purchase or Interlibrary Loan of an item not found in the Public Library Services catalogue.

Community services
- Adult literacy material - bulk loan request new window, external service - Request bulk loans of adult literacy materials.
- Languages other than English (LOTE) - bulk loan request new window, external service - Request loans of community language materials.
- Resource request - bulk loan new window, external service - Request bulk loans of talking books and storytelling resources from PLD.
Country Lending Service (CLS)
- Exchange selection form - new window, external service This is for the use of Country Lending Service libraries for the selection of exchanges.
- Checklist for the return of exchanges - List of items to check before you send your exchanges back to PLD.
- Directory assistance - This form is for the use of Queensland public library staff editing content for their library service in the Directory of Queensland public libraries.
- Directory errors new window, external service – This form is used to report incorrect information or provide comments on the content of the Directory of Queensland public libraries
- Lost and damaged book report - Report PLD books which have been lost or damaged by borrowers during the last month.
Database authentication (barcode ranges and IP addresses)
- Changes/edits to IP/Barcode ranges - State Library of Queensland needs this information in order to ensure that access to the databases for public libraries in Queensland is as seamless as possible for you and your clients. We will ensure that database vendors are advised of any updates.
